First Visit
Your initial appointment will usually consist of a comprehensive examination, and review of your treatment options. Usually, treatment can begin the same day as your consultation. However, a complex medical history or treatment plan may require an evaluation and second appointment, to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, your current ID card / subscriber ID.
- Your completed Patient Registration Forms.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please let us know prior to treatment, if you have a medical condition that may be of concern (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Transferring your records:
If your prior dentist or physician has current x-rays on file, you may request those records be released to our practice. Often, digital radiographs can be sent to us via email attachment; traditional xrays can be sent via US mail. If your existing x-rays are not current, updated digital imags can be taken at our office.
